This document specifies requirements and gives guidance on the procedures in the preparation of samples and the selection of reference materials for medical device testing primarily in biological test systems primarily in accordance with one or more parts of the ISO 10993 series. Specifically, this document addresses the following: — test sample selection; — selection of representative portions from a medical device; — test sample preparation; — experimental controls; — selection of, and requirements for, reference materials; — preparation of extracts. This document is not applicable to live cells but can be relevant to the material or medical device components of combination products containing live cells. Extractions for chemical characterization are covered in ISO 10993-18. Clause 7, 8, 9, 10 [with the exception of 10.3.5 and 10.3.11 b)], and 11 can apply to extractions for chemical characterization. Information given in C.1 to C.4 can also be relevant.
Abstract
Overview
ISO 10993-12:2021 sets internationally recognized requirements and guidance for the preparation of samples and the selection of reference materials for the biological evaluation of medical devices. As an essential part of the ISO 10993 series, this standard ensures that medical device testing-particularly in biological systems-is reliable, reproducible, and consistent across laboratories and jurisdictions. This document does not apply to the direct use of live cells but remains relevant for materials or components of combination products containing live cells. Detailed methodologies are outlined to enable manufacturers, testing laboratories, and regulatory bodies to produce valid and comparable results, supporting patient safety and device quality worldwide.
Key Topics
-
Test Sample Selection
Guidance is provided on how to select representative test samples from a medical device, ensuring all materials and processes are appropriately represented in biological testing. -
Sample and Reference Material Preparation
The standard describes procedures for preparing both test samples and reference materials (RMs), including handling of clean, sterile, or disinfected items, and preventing contamination during preparation. -
Experimental Controls
The proper use of experimental controls (positive, negative, and blanks) is defined to verify test system performance and support the reliability of biological evaluations. -
Reference Materials
Criteria for selecting, certifying, and using reference materials are provided. The standard recommends that reference materials be homogeneous, stable, and appropriately characterized for their intended use. -
Preparation of Extracts
Comprehensive extraction procedures are outlined, covering selection of appropriate vehicles (polar and non-polar), extraction conditions (temperature, duration), and treatment of both absorbable and non-absorbable device materials.
Applications
ISO 10993-12:2021 is critical in a range of medical device development and regulatory scenarios:
- Manufacturers use the standard to prepare samples for biological evaluation, ensuring products comply with regulatory requirements in global markets.
- Testing Laboratories follow the standardized procedures to guarantee consistency and comparability of test results, especially important for multi-site studies and international submissions.
- Regulatory Bodies reference ISO 10993-12 to assess the adequacy of clinical data and sample preparation methods in device submissions.
- Quality Assurance Teams utilize the standard to support risk management processes and document control through the appropriate selection and maintenance of reference materials.
Proper implementation of ISO 10993-12 harmonizes sample preparation and analysis protocols, minimizes variability, and enhances biological safety assessments in compliance with the broader ISO 10993 series for biocompatibility.
Related Standards
ISO 10993-12:2021 is part of a comprehensive framework for the biological evaluation of medical devices, working closely with other elements of the series and related standards, including:
- ISO 10993-1: Evaluation and testing within a risk management process.
- ISO 10993-5: Tests for in vitro cytotoxicity (extraction method details for cytotoxicity testing).
- ISO 10993-18: Chemical characterization of materials (covers extraction for chemical analysis).
- ISO/TS 10993-19: Physical and chemical characterization of materials.
- ISO 17034 and ISO Guide 35: Reference material production and use.
- ISO/IEC Guide 99: International vocabulary of metrology (relevant definitions).
These interconnected standards form the backbone of biocompatibility and risk management for medical devices, ensuring safe and effective performance in clinical use.
