ISO 13408-7:2012 specifies requirements and provides guidance on alternative approaches to process simulations for the qualification of the aseptic processing of medical devices and combination products that cannot be terminally sterilized and where the process simulation approach according to ISO 13408-1 cannot be applied. ISO 13408-7:2012 describes how risk assessment can be used during the development of an aseptic process to design a process simulation study for medical devices and combination products in those cases where a straightforward substitution of media for product during aseptic processing is not feasible or would not simulate the actual aseptic process.
Abstract
Overview
ISO 13408-7:2012 - Aseptic processing of health care products, Part 7 - provides requirements and guidance for alternative process simulations used to qualify aseptic processing of medical devices and combination products that cannot be terminally sterilized and for which the conventional ISO 13408-1 media-substitution simulation is not practicable. The standard explains how risk assessment drives the design, execution and documentation of process simulations throughout product development, scale-up and commercial validation.
Key topics and requirements
- Scope: Applies when full substitution with sterile liquid media or direct media contact would not realistically simulate the actual aseptic process.
- Risk-based strategy: Requires documented risk assessment to identify contamination risks, define acceptable contamination levels, and select mitigation/monitoring measures.
- Process simulation strategy: Documented approach that covers the entire aseptic process (or defined sub-processes), with rationale where conventional simulation is infeasible.
- Lifecycle approach: Initial simulations before first‑in‑human clinical production, adaptations during scale-up, and dedicated validation for commercial production.
- Testing and monitoring: Guidance on media selection, growth support, simulation procedures, incubation/inspection of units, sterility testing and investigation of positive units.
- Qualification cycles: Initial performance qualification, periodic requalification and repeat qualification when necessary.
- Documentation: Full record of strategy, risk assessments, simulation design and results to demonstrate contamination control.
- Informative annexes: Annex A (risk assessment example and quality risk management), Annex B (sample selection for microbial testing), Annex C (testing options for process simulation).
Practical applications
- Validating aseptic manufacturing for products such as:
- drug‑coated or biologically coated implants (e.g., stents, bone graft materials)
- bioprostheses and biodegradable implants
- wound dressings containing biologics or drugs
- combination product kits containing biological components
- hermetically sealed or partially enclosed electronic/implantable devices
- Designing surrogate-product strategies and targeted microbiological studies where direct media substitution is impossible.
- Using quantitative monitoring and modeling to estimate contamination risk and verify mitigation effectiveness.
- Supporting regulatory submissions and demonstrating aseptic process control for clinical and commercial batches.
Who should use this standard
- Sterile process validation engineers and microbiologists
- Quality assurance / regulatory teams in medical device and combination product manufacturers
- R&D and clinical development teams scaling aseptic processes
- Contract manufacturers (CMOs) performing aseptic filling or assembly of non‑terminally sterilized products

Using ISO 13408-7 helps manufacturers implement a defensible, risk‑based approach to aseptic process simulation and validation when conventional media-substitution methods are not feasible.
