ISO 21439:2009 specifies methods for the determination of absorbed-dose distributions in water or tissue that are required prior to initiating procedures for the application of beta radiation in ophthalmic tumour and intravascular brachytherapy]. Recommendations are given for beta-radiation source calibration, dosemetry measurements, dose calculation, dosemetric quality assurance, as well as for beta-radiation brachytherapy treatment planning. Guidance is also given for estimating the uncertainty of the absorbed dose to water. ISO 21439:2009 is applicable to “sealed” radioactive sources, such as plane and concave surface sources, source trains of single seeds, line sources, shell and volume sources, for which only the beta radiation emitted is of therapeutic relevance. The standardization of procedures in clinical dosemetry described in ISO 21439:2009 serves as a basis for the reliable application of beta-radiation brachytherapy. The specific dosemetric methods described in ISO 21439:2009 apply to sources for the curative treatment of ophthalmic disease, for intravascular brachytherapy treatment, for overcoming the problem of restenosis and for other clinical applications using beta radiation. ISO 21439:2009 is geared towards organizations wishing to establish reference methods in dosemetry aiming at clinical demands for an appropriately small uncertainty of the delivered dose. ISO 21439:2009 does not exclude the possibility that there can be other methods leading to the same or smaller measurement uncertainties.
Abstract
Overview
ISO 21439:2009 - Clinical dosimetry - Beta radiation sources for brachytherapy - specifies recommended methods for determining absorbed-dose distributions in water or tissue prior to clinical use of beta-emitting brachytherapy sources. The standard focuses on reliable dosimetry for short-range beta radiation used in ophthalmic brachytherapy and intravascular brachytherapy, including calibration, measurement, dose calculation, quality assurance and uncertainty estimation for sealed beta sources.
Key topics and technical requirements
- Scope of sources: Applicable to sealed beta sources (plane/concave surface sources, source trains, line, shell and volume sources) where beta emission is therapeutically relevant.
- Calibration and traceability: Guidance on measurands, traceability chains, primary/secondary/transfer standards and calibration procedures to ensure consistent dose delivery.
- Dose measurement methods: Procedures for in-phantom measurements in water or water-equivalent phantoms, detector selection (small-volume detectors required because of steep dose gradients), and corrections for detector response.
- Dose calculation and formalisms: Parameterization of radiation fields, radial dose profiles, normalization of relative dose for seed sources, and adaptation of established formalisms (e.g., TG-43/60) for long beta line sources.
- Theoretical modelling: Use of point-dose kernels and Monte Carlo simulation techniques for source characterization and treatment planning support.
- Uncertainty analysis: Guidance for estimating uncertainties of absorbed dose to water, including uncertainties from primary/secondary standards, transfer standards, measurements and modelling.
- Clinical quality control: Recommendations for acceptance testing, constancy checks and documentation required for safe clinical implementation.
- Reference data and annexes: Informative annexes provide reference data sheets, detector/phantom guidance, primary standards and Monte Carlo examples.
Practical applications and users
ISO 21439:2009 is intended for professionals and organizations involved in beta brachytherapy implementation:
- Medical physicists establishing clinical dosimetry programs for ophthalmic or intravascular treatments
- Hospitals and radiotherapy departments implementing beta source treatments and QA procedures
- Calibration laboratories and standards institutes developing traceable calibration services
- Manufacturers of sealed beta sources and dosimetry equipment preparing product specifications and documentation
- Regulatory bodies seeking harmonized dosimetry practices for licensing and inspection
Following ISO 21439 helps minimize dose uncertainty, improve treatment planning accuracy and support consistent clinical outcomes for beta-radiation brachytherapy.
