This document specifies requirements for hip-joint replacement implants. With regard to safety, this document specifies requirements for intended performance, design attributes, materials, design evaluation, manufacture, sterilization, packaging, information supplied by the manufacturer and methods of test. This document applies to both total and partial hip joint replacement implants. It applies to components made of metallic and non-metallic materials. This document applies to a wide variety of hip replacement implants, but for some specific hip replacement implant types, some considerations, not specifically covered in this document, can be applicable. Further details are given in 7.2.1.2. The requirements which are specified in this document are not intended to require the re-design or re-testing of implants which have been legally marketed and for which there is a history of sufficient and safe clinical use. For such implants, compliance with this document can be demonstrated by providing evidence of the implant’s sufficient and safe clinical use.
Abstract
Overview - ISO 21535:2023 (Hip-joint replacement implants)
ISO 21535:2023 specifies specific requirements for hip-joint replacement implants, covering both total and partial hip replacements and components made of metallic and non‑metallic materials. As a level‑3 standard for non‑active surgical implants, it addresses safety and intended performance across the product life cycle: design attributes, materials, design evaluation, manufacture, sterilization, packaging, labelling, and test methods. The standard also recognizes implants with an established history of safe clinical use and allows demonstration of compliance via clinical evidence.

Key Topics and Technical Requirements
-
Intended performance and design attributes
- Requirements for tolerances, dimensions and connections (including taper interfaces), articulating surface geometry and surface finish.
- Thickness guidance for acetabular components, bipolar and dual‑mobility heads (including UHMWPE liners and metal/ceramic shells).
-
Materials and biocompatibility
- Applies to metallic, ceramic and polymeric materials (e.g., UHMWPE) and references biological evaluation standards for material safety.
-
Design evaluation and pre‑clinical testing
- Extensive bench testing and pre‑clinical evaluations, including wear, fatigue and deformation considerations.
- Rules for testing the “worst case” configuration and procedures when performance requirements are not specified or not met.
- New requirements for when clinical investigations are needed and for post‑market surveillance.
-
Manufacture, sterilization and packaging
- Expectations for production control, validated sterilization methods and packaging that preserves sterility and component integrity.
-
Information for users
- Required labelling and instructions for the surgeon and patient, including warnings about component positioning and range of motion; electronic instructions for use permitted in some jurisdictions.
Applications - Who Uses ISO 21535:2023
- Medical device manufacturers - product design, verification, regulatory submissions and technical documentation for hip implants.
- Test laboratories and QA teams - bench testing, worst‑case evaluation and conformity assessment.
- Regulatory bodies and notified bodies - assessing compliance for market approval.
- Clinicians and hospital procurement - understanding device specifications, safe use instructions and device compatibility.
- Post‑market surveillance teams - establishing monitoring and reporting protocols.
