This method includes only bone deficiencies due to the failure of the formation ability of segments. It bases on anatomic and radiologic statements while ethiologic and epidemiologic factors are excluded. An example for the description of longitudinal deficiencies is given in the annex.
Abstract
Overview
ISO 8548-1:1989 - Prosthetics and orthotics - Limb deficiencies - Part 1: Method of describing limb deficiencies present at birth - specifies a standardized, anatomical and radiologic method for describing congenital bone deficiencies of the upper and lower limbs. The standard applies only to bone deficiencies caused by failure of formation of segments and explicitly excludes etiologic and epidemiologic factors. It provides a simple, consistent classification into transverse and longitudinal deficiencies and includes illustrative examples (Annex A).
Key topics and requirements
- Scope: Describes congenital bone formation failures only (anatomic/radiologic basis); excludes cause and epidemiology.
- Primary classification:
- Transverse deficiencies - limb develops proximodistally to a level beyond which no skeletal elements exist (record side, limb, and level of termination).
- Longitudinal deficiencies - one or more bones partially or totally absent (record side, limb, names of affected bones, and extent).
- Detailing longitudinal deficiencies:
- Identify affected bones (shoulder/pelvic girdles, long bones, carpus/tarsus, metacarpals/metatarsals, phalanges).
- State which bones are totally absent and which are partially absent; for partial absence, indicate approximate fraction and position (proximal/distal).
- Numbering conventions for digits and rays are provided; optional further description (e.g., naming phalanx) may be added.
- Additional requirements: Note presence of hypoplasia when applicable. Use standard vocabulary per normative references (ISO 8549-1 and ISO 8549-2).
- Informative Annex A: Provides example descriptions to guide consistent reporting.
Applications and users
Who benefits:
- Prosthetists and orthotists - consistent limb-deficiency descriptions to guide prosthetic/orthotic design and fitting.
- Clinicians and surgeons (orthopedics, pediatrics, radiology) - standardized documentation in clinical records and imaging reports.
- Rehabilitation teams - multidisciplinary communication and treatment planning.
- Manufacturers and design engineers - specification inputs for custom devices and component selection.
- Researchers and registries - harmonized case descriptions for comparative studies (note: epidemiologic analysis requires external classification systems). Practical uses:
- Clinical documentation and handover
- Device prescription and component selection
- Comparative reporting across centers and international exchange
- Training and quality assurance in prosthetics and orthotics
