ISO 10993-3:2014 specifies strategies for risk estimation, selection of hazard identification tests and risk management, with respect to the possibility of the following potentially irreversible biological effects arising as a result of exposure to medical devices: genotoxicity; carcinogenicity; reproductive and developmental toxicity. ISO 10993-3:2014 is applicable when the need to evaluate a medical device for potential genotoxicity, carcinogenicity, or reproductive toxicity has been established.
Abstract
Overview
EN ISO 10993-3:2014 (adopted by CEN as EN ISO 10993-3:2014) is part of the ISO 10993 series for the biological evaluation of medical devices. It specifies strategies for risk estimation, selection of hazard-identification tests and risk management related to potentially irreversible effects from exposure to medical devices: genotoxicity, carcinogenicity, and reproductive & developmental toxicity. The standard applies when evaluation of a device for these specific endpoints has been deemed necessary.
Key topics and requirements
- Test strategy and risk estimation
- Principles for deciding when genotoxicity, carcinogenicity or reproductive toxicity testing is required.
- Flowchart-based approaches (see informative annexes) for follow-up evaluation.
- Genotoxicity testing
- Guidance on selecting appropriate in vitro and in vivo genotoxicity test methods.
- Requirements for sample preparation and extract selection to ensure relevant exposure conditions.
- Carcinogenicity testing
- Additional requirements and evaluation strategies specific to long-term carcinogenic risk.
- Considerations for implantation or site-specific studies (normative annex).
- Reproductive and developmental toxicity
- Test strategy tailored to reproductive/developmental endpoints, including sample preparation and recommended test approaches.
- References to in vitro embryo-toxicity methods (informative annex).
- Sample preparation and reporting
- Annex A provides guidance on sample-preparation procedures for genotoxicity testing.
- Required content of the test report to support regulatory review and risk management.
- Supporting annexes
- Flowcharts, rationale for test systems, cell transformation systems, implantation study considerations, and bibliography for further method detail.
Practical applications and users
EN ISO 10993-3:2014 is used by:
- Medical device manufacturers for preclinical safety assessment and design validation when materials or device chemistry raise concerns about genotoxicity, carcinogenicity or reproductive toxicity.
- Regulatory affairs and clinical safety professionals preparing technical documentation for conformity assessment (note: presumption of conformity links with ISO 10993-1 and relevant EU Directives).
- Toxicologists and biological testing laboratories selecting appropriate test batteries, sample extraction procedures and interpreting results.
- Notified bodies and auditors assessing device safety dossiers for approval or market surveillance.
Related standards
- ISO 10993-1 (general principles for biological evaluation) - compliance with ISO 10993-1 is complementary and often required.
- Relevant national/adopted versions: EN ISO 10993-3:2014 (CEN adoption), and other parts of the ISO 10993 series covering cytotoxicity, sensitization, systemic toxicity, etc.
