This document specifies requirements for single use, gravity feed infusion sets for medical use in order to ensure their compatibility with containers for infusion solutions and intravenous equipment. Secondary aims of this document are to provide guidance on specifications relating to the quality and performance of materials used in infusion sets and to present designations for infusion set components.
Abstract
Overview - ISO 8536-4:2019 (Infusion sets for single use, gravity feed)
ISO 8536-4:2019 is the sixth-edition international standard that specifies requirements for single-use, gravity-feed infusion sets used in medical care. Its primary aim is to ensure compatibility of infusion sets with containers for infusion solutions and with intravenous equipment, and to provide guidance on material quality, performance and component designations. This edition updates physical requirements (notably air-inlet devices), labelling, leakage and flow-rate test methods compared to the 2010 edition.
Key topics and technical requirements
The standard covers technical and quality aspects across design, materials, testing and labelling. Major topics include:
- General design and nomenclature - standard component names and example configurations (vented and non‑vented sets, stand‑alone air‑inlet devices).
- Materials - requirements for materials in contact with fluids and the overall construction of the set.
- Physical performance (Clause 7) - includes:
- Particulate contamination limits and manufacturing cleanliness.
- Leakage testing to confirm airtight performance.
- Tensile strength (set must withstand minimum static tensile force - see Clause 7.3).
- Closure‑piercing device dimensions and no‑coring requirement.
- Air‑inlet device requirements including air filters to prevent microbial ingress and design to avoid air entrainment.
- Tubing, fluid filters, drip chamber/tube, flow regulators, flow‑rate determination, injection sites and fittings.
- Chemical and biological requirements (Clauses 8–9) - extractables/impurities, metal ions, sterility, pyrogenicity, haemolysis and toxicity testing.
- Labelling, packaging and disposal (Clause 10–12) - unit and multi‑unit labelling requirements, protective caps, and packaging expectations.
- Normative test methods - Annexes A–C contain physical, chemical and biological test procedures.
Practical applications and users
ISO 8536-4:2019 is used by:
- Medical device manufacturers designing and producing single‑use gravity infusion sets and stand‑alone air‑inlet devices.
- Quality assurance and regulatory teams ensuring product compliance, sterility and safety.
- Test laboratories performing particulate, leakage, tensile, chemical and biological testing per the annexes.
- Hospitals, procurement and clinical engineering for product selection and safe use guidance.
- Design engineers working on compatibility with infusion containers and small‑bore connectors.
Adherence improves patient safety, reduces infection risk and supports regulatory submissions and international market access.
