This document establishes a method of describing and measuring lower limb amputation stumps. It also lists the measurements required for the provision of a prosthesis.
Abstract
Overview
ISO 8548-2:2020 - Prosthetics and orthotics - Limb deficiencies - Part 2: Method of describing lower limb amputation stumps - defines a standardized method for describing and measuring lower limb amputation stumps. The standard establishes common descriptors, reference levels and planes, and the set of measurements required to support clinical assessment, rehabilitation and the provision of a prosthesis. This second edition revises stump descriptors and the measurement lists used by the rehabilitation team and by prosthetists.
Key technical topics and requirements
- Scope and purpose: Standardizes stump description and measurement so outcomes and fittings can be compared across teams and settings.
- Stump descriptors: Systematic recording of stump characteristics such as shape (conical, bulbous, cylindrical), soft tissue amount/consistency, muscle attachment, bony features and end‑bearing status.
- Skin, circulation and pain: Record skin barrier integrity, sensation, scar status, circulation (colour, temperature, oedema), significant pain and phantom sensations/pain using appropriate scales.
- Joint and muscle assessment:
- Range of motion recorded with the neutral zero method.
- Muscle strength recorded with manual muscle testing (0–5 scale).
- Joint stability and joint pain documented.
- Reference levels and planes: Defines anatomical reference levels (e.g., waist, iliac crest, proximal, inflection, stump end, femoral condylar, medial joint line, malleolar) and planes for partial foot amputations (heel, anterior tibial, stump end, toe).
- Measurements required: Specifies which length, circumferential, width and antero‑posterior measurements should be recorded for each amputation level (illustrated in figures). The document specifies what to measure, not the specific measuring technique. It also distinguishes measurements to be taken by the multidisciplinary team and those taken only by the prosthetist.
Practical applications and users
ISO 8548-2:2020 is intended for multidisciplinary use in clinical practice, research and public health:
- Prosthetists - to obtain the standardized measurements needed for socket and prosthesis design and fitting.
- Surgeons and rehabilitation physicians - to document stump status pre/post surgery and during follow‑up.
- Physical and occupational therapists, nurses - to record functional and skin/circulation status relevant to prosthetic use.
- Researchers and epidemiologists - to compare outcomes across centres using consistent stump descriptors and measurement sets.
- Healthcare administrators and policy makers - for quality assessment and service planning.
Using ISO 8548-2 helps improve communication among clinicians, supports reproducible prosthetic fitting, and enables meaningful outcome comparisons.
